Azimuth Precertification

Pre-notification is required for the following procedures:

  • Inpatient Admission,

  • Surgical procedures,

  • Labor/Delivery,

  • Chemo and/or Radiation Therapy,

  • MRIs, CT scans, or Pet Scans,

  • Air Ambulance and/or Emergency Medical Evacuations.

If you need any of the above procedures, you, your health care provider, or a friend/family member can initiate the pre-notification process by calling the Azimuth office at +1 317-644-6291 or +1 888-201-8850.

If you are calling after normal business hours or on the weekend, you may leave a detailed voicemail. Azimuth will also accept pre-notification via email at service@azimuthrisk.com. Use the Non-US Pre-notification form attached to this article. We recommend requesting precertification as early as possible.

Failure to provide pre-notification will result in a 50% reduction of eligible expenses. Pre-notification is not a guarantee of coverage.
